Effective December 1, 2019, there are new events and forms to request payment of Unclaimed Funds.

NOTE: Review the Unclaimed Funds Instructions thoroughly as there are revised requirements for the supporting documentation. -
Proposed Order Granting Application for Payment of Unclaimed Funds Form U-2a
The following forms are mailed to the Court "Attention Financial Administrator" (DO NOT file in CM/ECF):

Request for Payee Information and TIN Certificate Form AO 213P

Request for Taxpayer Identification Number and Certificate - IRS Form W9
All unclaimed funds paid into the Court pursuant to 11 U.S.C. Section 347(a) shall only be withdrawn by order of Court.
Any party desiring to claim funds may do so by filing: (1) Form U-1a Notice of Motion and Proof of Service, (2) Application for Payment of Unclaimed Funds Form U-1b 1340, (3) Proposed Order Granting Application for Payment of Unclaimed Funds Form U-2a, and (4) Unclaimed Funds Supporting Documentation.
The applicant must serve a copy of the application and supporting documentation to the United States Attorney and other parties as outlined in the Unclaimed Funds Instructions.
Note: All Motions are scheduled in front of Chief Bankruptcy Judge Jacqueline P. Cox. Hearing time is at 1:30 p.m. on a Tuesday when the Judge is sitting.
After filing the notice of motion, application and proposed order, file the Unclaimed Funds Supporting Documentation using the event: Bankruptcy>Other>Unclaimed Funds Supporting Documentation
Note: Closed Cases do NOT need to be reopened when this motion is filed.
Step-by-Step Procedure:

Select [Bankruptcy>Motions/Applications]

Enter case number and click Next

Select Payment of Unclaimed Funds from pick list and click Next

Select Party from party pick list or Add/create new party, click Next

The Attorney/Party Association screen may display. If it does, click here. If not, Click Next, and go to step 6.

The following message will appear:
- Click Next
- The PDF Document selection screen will display
- Browse, View and Attach the Notice of Motion and Application - the proposed order must be added as an attachment to the Notice of Motion and Application

Click Yes radio button for Attachment to Document and click Next

Attach Proposed Order, click Next

The following message will appear:
IMPORTANT: Please schedule this motion before Chief Judge Cox, on Tuesdays at 1:30 p.m. on a date when the Judge is sitting. Select the location: Appear in Courtroom 680, 219 S. Dearborn St, Chicago, IL 60604 or using Zoom-Judge Cox.
